A Cliniccal Study of Diagnostic Accuracy of Preoperative Diagnostic Methods in Patients with Gastric Cancer

Abstract
A Clinical study was carried out on 531 cases of gastric cancer operated during the seven years period between January 1987 and December 1993 at the Department of Surgery, National Medical Centel. They undeerwent preoperative diagnostic studies
including gastrofiberscopy, upper gastrointestinal series, abdominal uoltrasonography, abdominl computed tomography(CT). Authors analysed diagnostic accuracy and cost effectiveness of preoperative diagnostic methods.
1) There was no difference of diagnostic aaccuracy between gastrointestinal and upper gastrointestinal series in detection of cancer and determination whether EGC or AGC.
2) Gastrofiberscopic biopsy was essential despite of grossly benign lesion.
3) The diagnostic accuracy of abdominal CT for lymph nodes status was better than abdominal ultrasonography.
4) For the preoperative detection of inoperable intraabdominal metastatic gastric cancer, abdominal ultrasonography was more valuable than abdominal CT.
In conclusion, gastrofiberscopy with biopsy and abdominal ultrasonography were adequate for preoperative diagnostic tools of gastric cancer in the view of cost-effectiveness.
